St. Jude Medical uses an online submission process to objectively evaluate each professional education application we receive. If you wish to request a grant, please select the appropriate link below the Grant Application Types heading.

To submit a grant application, select the link below and you will be taken to the Web site for the online application system. The online application system will require you to upload one or more of the following documents with your grant applications:
- Request letter on your organization's letterhead. The request letter must include the following information:
- Grant purpose
- Amount requested
- How the grant will be used
- Official invitation, program or agenda
- Estimated total budget for the program, activity or event

Note: Be sure to include a physical mailing address. P.O. boxes are not acceptable. Additional documents are required for some of the grant types. For example, professional education programs offering continuing education credits may require a standard letter of agreement. Fellowship applications require a CV, prospectus and breakdown of salary and fringe.
Remember – We must receive a complete application, including all required documentation, at least 45 days before the event, activity, or program.
Grant Application Types
Fellowship Applications
Includes: Accredited fellowship programs in the cardiac and pain management arenas. St. Jude Medical does not support multi-year requests and each funding year must be applied for annually. To manage our budget and to assure we can fund as many worthwhile programs as possible, we limit funding for fellowships to once per budget year.
Requirement: The program must be related to cardiac and chronic pain disease states, conditions, and treatments for which St. Jude Medical products are used. Additional required documents to include:
- CV
- Prospectus
- Breakdown of budget by salary and fringe
Please Note: This application must be submitted directly by the organization. Do not include the fellow's name in the application. The requesting organization has sole discretion for who they choose as a recipient of the grant.

Grand Round Applications
Includes: Grants for a grand rounds program that is related to cardiac and chronic pain disease states, conditions, and treatments for which St. Jude Medical products are used.
Requirement: Grand round programs must be submitted once per event. The SJM Grant Program does not fund year-long or ongoing non-fellowship educational programs

Professional Education Applications
Includes: Independent medical educational programs for health care professionals (physicians, nurses, etc.) that offers continuing education credits. Also, congresses, conferences, and symposia sponsored by professional associations, societies, or continuing education providers.

Professional Educational Attendance for HCPs-in-Training Applications
Includes: Grants to educate health care professionals-in-training regarding cardiac and chronic pain disease states, conditions, and treatments for which St. Jude Medical products are used. Grants are not available for health care professionals who are not in training to attend independent medical education programs or to attend board examination preparation courses.
Please Note: Do not use this application for a request that would qualify as Professional Education. This application must be submitted directly by the organization. Do not include the HCP-in-training/fellow name in the application. The requesting organization has sole discretion for who they choose as a recipient of the grant.
