St. Jude Medical will provide a Returned Products Kit to organizations and physicians who have explanted devices from patients. This kit contains the following items:

• Postage-paid explant box with shipping label
• Removed Device Information form
• Biohazard bag
• Biohazard labels to seal the explant boxes
