Notice of Electronic Disclosure of Protected Health Information in accordance with Texas HB 300

St. Jude Medical cares about the privacy of your Protected Health Information (PHI). PHI may include information that relates to: 

  • Your past, present, or future physical or mental health or condition; 

  • Healthcare provided to you including device implants, or patient monitoring; and 

  • Past, present, or future payment for your healthcare. 

St. Jude Medical puts forth its best efforts to comply with all applicable laws related to the protection of your PHI. Under applicable law, St. Jude Medical is only permitted to disclose your PHI electronically without your authorization in certain situations. For example St. Jude Medical may electronically disclose your PHI without your authorization for the following reasons: 

  • Providing information to a healthcare provider in emergency situations or if you see a health care provider that is not your regular healthcare provider; 

  • To provide technical or customer service to a health care provider who is treating you; 

  • To print and send you device registration cards as required by the FDA; 

  • To track, analyze and report product complaints to the FDA or other regulatory agencies as required by law; 

  • To service equipment when St. Jude Medical has a contract with your healthcare provider for St. Jude Medical™ products or services or when St. Jude Medical owns the equipment; 

  • Exchanging device implant information with other companies where St. Jude Medical is required by the FDA to track implanted devices; and 

  • To provide requested information to your health care provider about your implanted device(s) or remote monitoring.
